This Policy applies, in general, to all Internet sites operated by or on behalf of Q-State. Q-State is not responsible for how other web sites treat your privacy once you leave the Q-State site.
Information You Provide
Q-State collects information you knowingly and voluntarily provide to us, such as information you provide when signing up to receive e-mail alerts, completing a survey, making data entries in connection with the use of any web-based Q-State services to you, or when asking us a question or providing feedback. This information may include, personally identifiable information (i.e., your name, telephone number, e-mail, or health or medical information). You will be informed what information is required and what information is optional.
Q-State is committed to protecting the privacy of children. We do not knowingly collect personally identifiable information from a child under the age of 13.
Information Collected Passively
When you visit the Q-State website, certain anonymous information may be passively collected through the use of Internet tags, or other navigational data collection including log files, server logs, or clickstream. Your Internet browser automatically transmits to our websites some of this anonymous information, such as the URL of the website you just came from and the Internet Protocol (IP) address and the browser version your computer is currently using. Remember, the only personally identifiable information Q-State collects is the information you voluntarily provide.
In addition, Q-State gathers aggregate data to improve the content of our websites, to customize our websites to your preferences, for our internal marketing and research purposes, to analyze statistics and trends, and for any other purpose specified.
California Do Not Track Disclosure
Q-State does not track its customers over time and across third party websites to provide targeted advertising and therefore does not respond to Do Not Track (DNT) signals. However, some third-party sites do keep track of your browsing activities when they serve you content, which enables them to tailor what they present to you. If you are visiting such sites, your web browser allows you to set the DNT signal so that third parties (particularly advertisers) know you do not want to be tracked.
Third parties that have content embedded on Q-State websites such as a social feature may set cookies on a user’s browser and/or obtain information about the fact that a web browser visited a Q-State website from a certain IP address. Third parties cannot collect any other personally identifiable information from Q-State’s websites unless you provide it to them directly.
Use and Disclosure of Information
Q-State will not sell or rent personally identifiable information to any third party for any purpose. In addition, Q-State does not permit third parties to collect your personally identifiable information from any Q-State site. Q-State may use the services of a third party to provide Q-State with support services in connection with the website and such parties may, from time to time, have access to your personally identifiable information to enable them to provide those services to Q-State. We require all such third parties to meet the same standards of data protection as Q-State, and they are prohibited from using the information for their own marketing purposes.
Q-State uses personally identifiable information only for the purposes for which you provide it and to communicate information to you (if requested), except that Q-State may use information entered into the Q-State Medication Diary to conduct research, development and project planning after any personal details from the information is redacted.
How We Protect Your Personal Information
Q-State is committed to protecting the security of your personal information. Q-State takes reasonable steps to protect your information as you transmit it to our site and to protect such information from loss, misuse, and unauthorized access, disclosure, alteration, or destruction. Please keep in mind that no Internet transmission is ever completely secure or error free, and Q-State cannot be responsible for breaches of security beyond our reasonable control.
